Description The importance of the kinematic computer navigation of knee endoprosthesis lies above all in the reduction of out-layers. This fact is important especially for beginning orthopaedic surgeons. The kinematic navigation should prevent from wrong resection of distal femur or proximal tibia. The navigation helps routinely also at the beginning of the surgery to distinguish the “right” valgus deformity from the “false” one. According to it we can choose the suitable surgical approach. In unicompartmental knee joint replacements it is possible to choose the right polyethylene inlay height by the help of the navigation so that the lower limb mechanical axis is restored as accurately as possible. In this way we avoid axis overcorrection into valgus in most cases and subsequent decompensation of the lateral compartment and later necessity of conversion on TKR. The goal of this work is to share our experience with the kinematic navigation system usage in knee unicomparmental and total replacement (miniinvasive approach or standart one). This manual advert to advantage and disadvantage usage of this computer system during this type of the operation and advent to importance of the digital planning of the size of components and the mechanic axis of the lower limb.
