Description The issues of arterial wall and arterial pulse wave characteristics in hypertension have been recently studied while the importance of systolic blood pressure and pulse pressure values for the evaluation of cardiovascular risk has been stressed. Systolic blood pressure is not identical in all areas of vessel course, thus the sphygmomanometric brachial blood pressure measurement cannot detect the central blood pressure changes. The non-invasive method of central aortic pressure estimation and vascular rigidity evaluation has been widely used in many areas. The relationship between peripheral systolic brachial pressure and central systolic blood pressure differs in various patients and depends on the arterial rigidity and the pulse wave form. In common practice the easiest approach is to evaluate the rigidity of a certain vessel course segment – it can be examined by pulse wave velocity (PWV) evaluation method. Different groups of antihypertensive drugs have a different impact on the arterial compliance and thereby on central blood pressure, in spite of the fact that they have the same impact on peripheral blood pressure. An ideal antihypertensive drug should decrease blood pressure and have a positive impact on the wall rigidity.
