Description The osteonecrosis of jaws is a phenomenon newly described in the connection with bisphosphonate therapy. We describe a case of a 41-year-old man with multiple myeloma who developed an osteonecrosis of the jaw in the course of anti-myeloma therapy. The man had been suffering from toothache since the beginning of the chemotherapy, nevertheless the extraction was not performed until one month before high dose chemotherapy with peripheral stem cell transplantation started by reason that the conservative therapy had proved unsatisfactory. Bisphosphonates were administered from the beginning of the chemotherapy; the bisphosphonate therapy was started with clodronate, which was later replaced by zoledronate. One year after performing the tooth extraction the wound stil would not heal up therefore resection of the necrotic bone was performed. The pain of the jaw diminished but the wound did not heal up completely. The administration of bisphosphonates was stopped after setting the diagnosis of osteonecrosis of the jaw.
