Description Introduction: Chronic otitis media (COM) is a very common middle ear disease in children and young adults. The main causes are Eustachian tube dysfunction and bacterial infections. The aim of this pilot study was to describe and compare middle ear bacteraemia in children and young adults with severe forms of COM, such as cholesteatoma and retraction pocket of the eardrum, with bacteraemia of the healthy middle ear. METHODS: In this case study, middle ear swabs were taken during surgery in children with cholesteatoma (N=23) or retraction pocket (N=26) and in children referred for cochlear implant (N=15, controls). Genomic DNA extraction was followed by 16S rDNA gene library construction and sequencing on the MiSeq instrument. Samples with a relative frequency of at least one bacterial genus >20% were considered positive for a specific genus. RESULTS: Bacterial diversity was generally low in middle ear samples from patients with COM, with DNA from one or two bacteria typically dominating the sample. A significant difference in positivity for one or more bacterial genera was observed between patients with a cholesteatoma or retraction pocket (38.8%) versus those indicated for a cochlear implant (6.7%). CONCLUSION: While the middle ear bacterioma in cases of cholesteatoma and retraction pocket differed from that of the control group, the findings in both pathologies were similar. These results support the claim that the retraction pocket is a stage before cholesteatoma.
